The Stylex Folding Camera is a simple camera for 620 film from the Herbert George company of Chicago, Illinois (later best known for the brand Imperial). It is a bit atypical among Imperial's 620 models in exposing rectangular 2¼×3¼" images rather than square ones. "Folding" is not quite an accurate name, as the lensboard is simply pulled straight forward before the camera can be used.
